  • Step 1


    Obtain Internal Revenue Service Form SS-4 by calling 1-800-829-4933 or by going to the IRS website (http://www.irs.gov) and searching for the term "SS-4." Choose from the website the latest revision of the form.





  • Step 2


    Fill out the one-page form entirely.





  • Step 3


    Resolve any questions you have about filling out the spaces on the form using the IRS form entitled "Instructions for Form SS-4," which is also obtainable by calling 1-800-829-4933 or by going to the IRS website (http://www.irs.gov) and searching for the term "Instructions for Form SS-4."





  • Step 4


    If your business is located in the United States, file your completed written Form SS-4 by mailing it to this address:

    If your business has no address or legal presence in the United States, file your completed written Form SS-4 by mailing it to this address:

  • Step 5


    Allow approximately 30 to 60 days for IRS notification of your business Federal Tax Identification Number.
